Accounting 131 One Minute Summary

 This is starter page for the form driven One Minute Summary survey for Acc131. Once you click on the One Minute Summary button a login screen will appear. The login screen is self explanatory for entering your name, id# and your email (if you want to see the results mailed to yourself). Starting the survey you will be presented with 10 questions which you must answer. At the bottom of the form is two buttons. The first "Review all answers" will allow you to review and change your answers. The second "Submit Survey" will return the unanswered questions. Once you have answered all of the questions (you must answer the text entry questions) it will inform you that you have done so and allow you to submit the survey for marking. Your answers will be summarized and shown to you and emailed to Professor Russell. If you fill in the email address on the log in page your answers will also be mailed to you.
