Participation in this project is voluntary and anonymous. You may omit any question you prefer not to answer, and you may choose not to submit any of our forms at any time without us collecting any of your information. Further, all information you provide will be considered confidential. At all times, the data collected through our research will be accessed only by the researchers involved, and will be maintained on a password-protected computer in a restricted access area at the University of Waterloo. All data collected through this research will be kept for a period of 5 years in a secure location and then destroyed.

As a participant in our research, you acknowledge that you have read the information presented in our website. You further acknowledge that you have had the opportunity to ask any questions related to this study, to receive satisfactory answers to your questions, and any additional details you may want. In return, we guarantee that any specific and potentially identifiable information you may provide on our attention lapse reports will not be shared with any outside sources without your permission.
